Nina Warken, the Federal Minister of Health (CDU), aims to maintain stable contributions to health insurance through the Primary Care System. This model proposes a shift towards visiting a family doctor initially, followed by referrals to specialist practices with exceptions for gynecologists, eye, and dental doctors. Beier voices concerns about a Swiss cheese-like structure with numerous exceptions and loopholes, arguing that most patients would continue to navigate the system independently in such a scenario.

The Primary Care System isn't just a referral-making machine. Family doctors, trained as generalists, tackle the majority of patient issues directly in their offices. Statistically, around 80 to 90 percent of all issues can be resolved at this level.

Andreas Gassen, chairman of the Federal Association of Statutory Health Insurance Physicians, proposes applying the Primary Care System only from the age of 50. He believes it is particularly beneficial for patients with multiple ailments, highlighting the need for a coordinating presence in the form of a family doctor's practice to consolidate all findings and make specialist referrals as necessary.

However, Armin Grau, the Green Party's health policy spokesman, criticizes this age-centric approach as ill-targeted. He asserts that health is not exclusive to the elderly and emphasizes the need for reliable primary medical care for the young as well. Grau also stresses the importance of supporting family doctors to prevent them from turning into mere referral points. He advocates strengthening their role as the first point of contact in the system through targeted relief and reduction in bureaucracy.
